Depth. Stormwater sits shallow. A line 1.5 to 2 metres down is almost certainly sewer, and if someone has quoted a dig that deep on a job they've called stormwater, get the plans checked first.
No. The gully has to sit above the surrounding ground so stormwater can't get into the sewer, and the grate is loose by design so it lifts when the line surcharges. Bury it and the next sewer blockage comes up inside the building. Paving, turfing and resurfacing crews cover these constantly on school and aged care sites, and the first anyone knows about it is the callout.
Council holds it and will supply the location, usually for a fee. Plenty of older Brisbane properties have no kerb outlet at all and run into a barrel drain under the street, a rubble pit, or a neighbour's line. Queensland stormwater drainage regulations and council plumbing approvals both key off that point.
Sometimes. Relining needs a host pipe intact enough to hold the liner and access at both ends, and a collapsed section still gets dug.
